3 mL (2.5 mL min.) Plasma from 1 Lavender Top (EDTA) tube. Patient should be ambulatory / upright 2 hours prior to the test. Separate plasma and freeze. Ship frozen. DO NOT THAW. Drug interactions to be noted: Potassium wasting diuretics, Spironolactone, Eplerenone, Amiloride and Triamterene should be discontinued at least for 4 weeks; Adrenergic blockers, Clonidine, Methyldopa, NSAIDs, Angiotensin-converting enzyme inhibitors, Angiotensin receptor blockers, Renin inhibitors and Dihydropyridine calcium channel antagonists should be discontinued for 2 weeks. If necessary to maintain hypertension control, patients should be treated with other antihypertensive medications like Verapamil slow-release, Hydralazine, Prazosin, Doxazosin & Terazosin that have lesser effects on Plasma renin & aldosterone levels. Any change in medication should be done in consultation with treating physician.

Doctors use the Aldosterone Plasma Renin Ratio (ARR) as a diagnostic tool to determine how the aldosterone levels match renin levels in a human body. The adrenal glands produce aldosterone hormone which helps manage sodium water levels in the blood along with potassium ions and blood pressure. The kidneys release the enzyme “renin” which plays a central part in managing both fluid levels and electrolytes within the body. The ARR test serves as a diagnostic screening measure to detect primary aldosteronism because this condition brings about elevated blood pressure and electrolyte imbalances.

Why Is an Aldosterone Plasma Renin Test Prescribed?

Doctors advise the aldosterone plasma renin test for patients who present hormones-related blood pressure symptoms. This test is vital in detecting primary aldosteronism (Conn's syndrome) because this adrenal gland disorder generates elevated aldosterone while reducing potassium and increasing hypertension levels in patients. 

The testing procedure is for patients with resistant hypertension conditions along with those who experience unexplained hypokalemia plus those with hereditary endocrine disorders. Monitoring hormone imbalances using the aldosterone plasma renin test enables early identification which stops risks like cardiovascular incidents and renal failure.

Components of an Aldosterone Plasma Renin Test

The ratio between aldosterone and renin in the blood is measured through the ARR test. The aldosterone direct renin ratio exceeds normal levels when the adrenal glands generate too much aldosterone. Ratio and accuracy of the test may be influenced by medications taken with diet, and sodium intake. Doctors suggest multiple confirmatory tests including salt-loading tests, adrenal CT scans, and adrenal vein sampling to identify the root cause of abnormal test results.

Procedure and Prerequisites

The results of the aldosterone plasma renin test require proper blood sample collection that must occur after resting. Patients are typically advised to:

  • Avoid using diuretics, beta-blockers, ACE inhibitors, and NSAIDs since these medications can cause testing errors.

  • Maintain proper sodium intake. Abrupt changes in sodium consumption affect the aldosterone hormone measurements.

  • Choose between sitting and lying posture during a specific period before extracting the blood sample.

Test results from blood analysis need several days to provide a satisfactory result. Further diagnostic tests are required to confirm the presence of primary aldosteronism or other underlying conditions when the aldosterone plasma renin test detects abnormal levels.
